- Patient Portal.When you sign up for a client portal through our electronic medical record (EMR) system, you can provide health records, medications, allergies, and health issues, view test and lab results, provide payment information to pay your bills, and the information you provided to create an account. We use an electronic medical record provider to maintain our portal.
- Pay a Bill. To pay a bill online, you will provide us with your account number, name, and date of birth.
- Request Submission Form. To submit a Client Request to Access Medical Records or Authorization for Use and Disclosure form, you will provide your name, date of birth, address, phone number, and signature.
- Sign Up for Newsletter. To sign up for our newsletter, you will provide us with your email address.
- Share Your Story. To share your story, you will provide your name, email address, and any information you choose to provide in your story.
- Volunteer or Apply for a Job. To volunteer or apply for a job, you will provide your name, phone number, email address, physical address, education information, employment information, and any other information you choose to provide. We use third parties to process your job applications. Please review each provider’s privacy policy for more information.
- Form 990 Request.To request a Form 990, you will provide your name, email address, and any other information you choose to provide.
- Contact Us.To contact us, you will provide your name, email address, phone number, address, and any other information you choose to provide.
Other data collected
We may collect other, non-personal information about your visit, including the pages you view, the links you click, and other actions taken in connection with your use of the website. UNIVERSAL OPT-OUT MECHANISMS
We do not engage in “sales” or “targeted advertising” as those terms are defined under applicable laws and therefore our Site does not recognize the Global Privacy Control (“GPC”) signal. UTBS never shares any information, other than your email address, with our third-party email vendor, which does not share these email addresses with anyone else.
If you receive information about a UTBS service through e-mail, you may make a request to discontinue receiving similar messages in the future by opting out of email notifications on your Client Portal.

This website may provide links to other websites that are not owned or controlled by us. We are not responsible for the privacy practices of other websites. MESSAGES AND TRANSACTIONS
Comments or questions sent to us using e-mail or secure messaging forms will be shared with UTBS staff and health care professionals who are most able to address your concerns. We archive all messages. When you use a service on the secure section of the website to interact directly with UTBS business departments or health care professionals, some information you provide may be documented in your medical record and available for use to guide your treatment as a client.
